Abstract
A new nursing history form, using Gordon’s functional health-pattern areas as a focus for data collection, was piloted on eight patient units in an acute-care setting. It was found that changing to a nursing perspective for the collection of patient information increased the documentation of valid nursing diagnoses by staff nurses.
