The problem-oriented record in psychiatry

Abstract

The Problem-Oriented Record (POR) was introduced by Dr. Lawrence Weed in 1968.1 However, it is only recently that hospitals and health agencies around the country have been converting to this type of patient record-keeping. Because the problem oriented model is new, it has not been part of the basic educational curriculum of most health professionals. Therefore, the transition from the traditional medical record to the POR must include an educational process that provides an informational framework which facilitates effective staff implementation of this approach.
